Two fatalities in somewhat similar circumstances could have been prevented, had ordinary care been exercised by those concerned. At Axwell Park Colliery Durham, an onsetter at a mid-inset contravened General Regulation 21 (a) by a premature removal of the shaft fence. He then pushed a full tub into the shaft, apparently under the mistaken impression that the cage had arrived at the inset, and fell with it. This strange lapse on his part was all the more inexplicable as the shaft inset was well lighted and the roadway rose slightly towards the shaft. In the other case at Garesfield "Bute" Colliery, Durham, a hand putter, who was also the attendant at the top of a staple, for some unexplained reason propped tip the shaft gate and forgot to lower it. When coming outbye with the next full tub he apparently saw the gate raised and wrongly assumed that the cage was holding it up. He pushed the full tub into the staple and fell with it.
