Description

At Harraton Colliery, Durham, an examiner standing in a damp roadway was renewing the fuses in a gate-end panel when he received a fatal shock from one phase of the A.C. 3 phase, 400 volt circuit. The panel was fitted with an interlocking device consisting of a cam on the switch spindle which, with the switch closed, obstructs the opening of the cover plate through a lug on one of the hinges. It is presumed that owing to the cam becoming slightly loose and the lug not completely covering the cam, deceased was able to open the panel with the switch closed. The gate-end panel was replaced by a better and safer design.
