The fourth case occurred on April 7th at Hummerbeck Colliery, belonging to Mr. Thomas Boddy, and caused the death of a hewer. Four men were riding in the cage and on its arrival at bank, three stepped out and the fourth was doing so when the engineman lowered the cage two or three feet, and the man's head was caught between the flat sheets and the top rim of the cage. No signal was given to the engineman, but he asserted at the inquest that he received one. The case was a bad one, and would not have occurred if the shaft had been fitted with "keps."
