No. 381 on the list occurred at West Auckland Colliery belonging to Messrs. Bolckow, Vaughan, & Co., Limited, on August 25th, causing the death of a deputy.
The deceased man was travelling on the engine plane to his work when he met the set and was caught by it, and so severely injured that he died the same day.
The refuge holes were within the distance required by the Act, and there was a space of 3 feet on one side of the way, and of 17 inches on the other.
His mind must have been preoccupied, and he had forgotten all about the set.
