Adams, Thomas, 09 Oct 1894, 9:50 a.m., 4th hour of shift, aged 45, Deputy, He was going down on the cage alone, having previously instructed the engineman to stop at the "eye." This was reached in safety, and a few minutes later the signal to "lower, men on" was received. He was found lying across the cage, on its arrival at the bottom, quite dead, his neck being dislocated [Inspection made & inquest attended]

      

Connor, Michael, 08 Mar 1894, (accident: 30 Nov 1893, 8:00 a.m., 2nd hour of shift), aged 53, Ore Picker, Deceased, when trying to stop a waggon and standing on the brake, was jammed between the top of the waggon and a post. He died on the 8th March 1894 [Inquiries made & inquest attended]

      

Connor, William John, 29 Nov 1886, aged 37, Banksman, crushed by water barrel ; the deceased was removing the water barrel, and had detached it from the rope on to a tram, and was pushing it away from the pit mouth, when he observed it tottering on the tram ; to prevent it from falling, he put his back to it, but owing to the slippery state of the flat sheets his feet slipped, and he fell back, the water barrel fell over and struck him on the chest, killing him instantly

      

Fearon, Henry, 25 Apr 1887, (accident: 06 Dec 1886), Sinker, injured by slipping of rod of boring machine causing fracture of leg ; as he was nearly well, the doctor thought it better to take a small piece of dead bone out of his leg, and administered chloroform for the purpose, from which he died

      

Hoast, John, 18 Sep 1885, aged 32, Miner, working under a bad roof, but well timbered close up; there had been a heavy fall on top of wood, causing it to give way suddenly; two men escaped

      

Nolan, Matthew, 27 Oct 1877, aged 28, Waterman, fell into sump from whence water was being drawn, his work being to steady the vat as the engine drew it

      

Ray, William, 30 Jun 1894, 10:30 a.m., 5th hour of shift, aged 27, Trailer, Fall down a staple pit. The front of this shaft was properly fenced, but the back was quite unprotected. The trailers were, it transpired, accustomed to lunch here, and in going round for this purpose, deceased slipped and fell down the pit [Inspection made & inquest attended]

      

Walters, Michael, 23 May 1892, 12:00 p.m., 6th hour of shift, aged 33, Miner, After firing a shot he commenced to get the loose ore down, when a large piece suddenly rolled off the side on to him. He had got on to the top of the timber to pull the ore, otherwise he would have been safe [Inspection made & inquest attended]
