Nevinson, Joseph Henry, 10 Oct 1912, aged 40, Sinker, the accident occurred in a sinking shaft during timbering operations; the empty tackle had been sent away and a portion of it, owing to careless attachment, fell back down the shaft and struck deceased on the head
