Ash, Michael, 12 Dec 1890, 7:30 a.m., 2nd hour of shift, aged 50, Miner, fall of roof stone; this accident occurred in a very high working, and it had not been visited by a deputy or an official for 23 hours previous to the accident

      

Bryan, James, 14 May 1897, (accident: 04 May 1897), aged 27, Miner, A dynamite shot had been fired in a lump of ore in the side of a robbery working. The shot failed to bring down the ore, and deceased was in the act of putting in another charge of dynamite when the piece of ore suddenly fell and caught him. He died on the 14th May [Inspection made & inquest attended]

      

Burns, John, 10 Jul 1889, aged 36, Miner, whilst engaged boring a hole, a piece of metal fell from a slip in the roof, striking deceased and causing injuries from which he died

      

Close, James, 04 Aug 1882, aged 20, Miner, killed by a fall of roof

      

Graham, John, 17 Dec 1913, aged 54, Miner, deceased had prepared a place for a charge of gelatine dynamite behind a piece of ore which was known to be unsound in the side of the working; he was just about to leave the working to prepare the charge when the lump, weighing about 2 tons, suddenly fell and caught him

      

Holmes, John, 21 Jan 1888, aged 26, Miner's Loader, fall of ore from the side; the miner in charge of this place had tried to bar the piece of ore down; and being unable to do so, he allowed the deceased to continue loading under it when it fell on him

      

Johnson, William, 10 Jan 1888, aged 36, Miner, while dressing shale off a cheek, a piece slipped off without the slightest warning, killing him; the overman was giving instructions to deceased at the time of the accident

      

Keough, Felix, 30 Aug 1895, 7:00 a.m., 1st hour of shift, aged 43, Miner, fall of ore and stone from the side of a high working; deceased heard some small pieces falling and jumped back, as he thought, out of the way, but, unfortunately he stepped immediately beneath the falling mass and was crushed to death; his mate, who was close beside him, was uninjured; the place had been officially examined about an hour previously, and reported safe

      

Killen, Edward, 29 Mar 1888, aged 26, Labourer, fall of ore from roof; the working where the deceased was killed was 24 feet high; the piece of ore which fell had been blasted by a shot, and was torn; one of the miners engaged in the place commenced to bore another hole in it, at the time warning the deceased that the piece was bad, and telling him to be careful; the whole mass came away, falling on him, killing him instantly

      

McVicar, David, 28 Nov 1882, aged 20, Miner, struck by stone from a shot

      

O'Pray, John, 13 Feb 1891, 10:30 a.m., 5th hour of shift, aged 42, Labourer, The piece of stone which fell was known to be bad two days previous to the accident. This occurred through gross negligence on the part of the officials in allowing the deceased to work under this dangerous stone [Inspection made & inquest attended]
